Athletic Clearance information and Eligibility Guide FHSAA

Welcome to Seminole High School, home of the Warhawks! Coaches, faculty and staff all look forward to a great 2026-2027 athletic season. While we know students are eager and excited to play, PCS and FHSAA require some things for all athletes to be able to participate. Please use the following guidelines to participate and access all required forms on the Athletic Eligibility Checklist below.
 
  1. Create your account. Go to athleticclearance.fhsaahome.org and set up an account for the 2026–2027 school year. Choose Seminole High School (Seminole) and the appropriate sport from the menu.
     
  2. Get your physical & ECG/EKG. Complete a sports physical on the state FHSAA EL2 form, signed by your physician. ECG/EKG's must be completed on the state FHSAA EL1 form.
     
  3. Purchase HSR K12 student insurance. Choose one of the options that fits your family's budget/needs.
     
  4. Complete your NFHS courses. Download a certificate for each of the following free courses.
     
  5. Upload everything. Upload all required documents (EL1 ECG/EKG, EL2 physical, NFHS course certificates, proof of insurance, and any additional required documents) to your Athletic Clearance account. All files must be in PDF format to prevent technical issues.
     
  6. Wait for review. Once everything is submitted, your status will say “Pending,” which means it is being reviewed. Students must be "Cleared" before participating in any athletic activities at Seminole High School.
